Obwohl die Bedeutung der Mobilisation und Bewegungstherapie für Patienten mit Neuropathia vestibularis bekannt ist, existieren derzeit in Deutschland keine standardisierten, evidenzbasierten Rehabilitationskonzepte. Das Anliegen dieser Längsschnittuntersuchung war die Entwicklung und Evaluierung eines sensomotorischen Trainingsprogramms.
In die Studie wurden 68 Patienten mit Neuropathia vestibularis eingeschlossen und bezüglich der Parameter Haltungs- bzw. Gleichgewichtsregulation sowie Lebensqualität untersucht.
Bei allen posturographischen Parametern konnten signifikante Zeiteffekte erzielt werden. Gruppeneffekte ließen sich nicht beobachten. Interaktionseffekte wurden beim Stabilitätsindikator (p=0,042) sowie in den Frequenzbereichen F 1 und F 5-6 ermittelt. Im Gesamtscore des DHI zeigten sich Zeit-, jedoch keine Interaktions- und Gruppeneffekte.
Bezüglich des visuellen, nigrostriatalen Systems (F 1) und des somatosensorischen Systems (F 5-6) ließen sich mit dem Training auf dem Aerostep größere Effekte erzielen als mit der traditionellen Rehabilitation. Die subjektive Befindlichkeit wurde sowohl durch die traditionelle Rehabilitation als auch durch das Aerostep-Training signifikant verbessert.
Das sensomotorische Training auf dem Aerostep erwies sich als geeignet, die Haltungsregulation von Patienten mit Neuropathia vestibularis nachhaltig zu verbessern.
Although the importance of mobilization and physical therapy is well known for patients with vestibular neuropathy, evidence-based rehabilitation concepts in Germany are still lacking. The subject of this cross-sectional study was the development and evaluation of a sensomotoric training program.
This study included 68 patients with vestibular neuropathy. The patients were examined with respect to the parameters posture and balance control and quality of life.
Significant time-related effects could be detected in all parameters investigated. Group effects were not observed. Interaction effects were found out in the stability indicator (p=0.042) and in the frequency ranges F 1 and F 5-6. The overall DHI score showed time-related, but no interaction or group effects.
The nigrostriatal system (F 1) and the somatosensoric system (F 5-6) showed a higher training-related effect with the aerostep than with traditional rehabilitation. The subjective vertigo score could be significantly improved with the aerostep and also with traditional rehabilitation.
Sensomotoric training with the aerostep proved to be well suited for improvement of posture regulation in patients with vestibular neuropathy.
